Dermal Fillers for Fat Pad Atrophy and Ball of Foot Pain

Pain beneath the ball of the foot is not always caused by a bone or joint problem. As the natural fat pad thins with age, there is less cushioning to absorb pressure during standing and walking. This can contribute to ball of foot pain, painful calluses, discomfort over bony prominences, and increased pressure in areas vulnerable to skin breakdown.

Following a comprehensive evaluation, our doctors determine the cause of your symptoms and recommend the most appropriate treatment. For carefully selected patients, dermal fillers may be used to restore temporary cushioning beneath painful or high-pressure areas of the foot, helping reduce pressure and improve comfort with standing and walking.

What are dermal fillers for the foot?

Dermal fillers are injectable materials used to temporarily add volume beneath areas of the foot where natural cushioning has been lost. They provide additional soft tissue padding that can help reduce pressure beneath painful or high-pressure areas during standing and walking.

Dermal fillers do not correct the underlying cause of fat pad atrophy or structural problems such as bunions and other bony prominences. They are one of several treatment options for pressure-related foot pain and are not appropriate for every patient. Before recommending treatment, our doctors perform a comprehensive evaluation to determine the cause of your symptoms and whether dermal fillers are appropriate for your diagnosis and treatment goals.

Why choose dermal fillers?

For carefully selected patients, dermal fillers offer a minimally invasive way to restore temporary cushioning to painful or high-pressure areas of the foot. The procedure is performed in our office under local anesthesia, requires no incisions or stitches, and allows patients to walk immediately after treatment with little disruption to normal daily activities.

Custom orthotics, shoe modifications, padding, and routine foot care may remain important parts of treatment. However, when loss of natural cushioning continues to cause pain despite conservative care, dermal fillers can provide an additional layer of protection between the skin and underlying bones. This may improve comfort with standing and walking and, in selected patients, help reduce pressure on areas vulnerable to skin breakdown.

Why choose Timonium Foot and Ankle Center for dermal fillers?

Dermal fillers for the foot are a specialized treatment that is not routinely offered by many podiatry practices. Successfully treating fat pad atrophy requires more than simply performing an injection—it requires an accurate diagnosis, careful patient selection, and an understanding of the underlying cause of your symptoms.

Dr. Jordan R. Stewart, DPM has performed hundreds of dermal filler procedures for the foot and has completed specialized training in dermal filler techniques. Every patient receives a comprehensive evaluation to determine whether dermal fillers are appropriate or whether another treatment, such as shoe modifications, custom orthotics, padding, or surgery, is more likely to provide lasting relief.

Dr. Stewart is Board Certified in Foot Surgery by the American Board of Foot and Ankle Surgery® (ABFAS), providing expertise in both conservative and surgical treatment options. Dermal fillers are recommended only when they are expected to benefit your specific condition and treatment goals. This individualized, diagnosis-first approach helps ensure that you receive the most appropriate treatment—not simply the newest or most aggressive procedure.

Dermal Filler, Answered
What conditions can dermal fillers help treat?
Dermal fillers may be recommended for carefully selected patients with fat pad atrophy, ball of foot pain (metatarslgia), painful plantar calluses, discomfort over prominent bony areas, or localized pressure beneath the foot. In some patients with diminished soft tissue padding, dermal fillers may also help reduce pressure on vulnerable areas of the skin as part of a comprehensive treatment plan. Every patient begins with a thorough evaluation to determine the cause of their symptoms before treatment is recommended.
Who is a candidate for dermal fillers?
Dermal fillers may be appropriate for patients whose pain is related to fat pad atrophy or localized pressure that has not responded adequately to conservative treatment. Candidates often continue to experience discomfort despite shoe modifications, padding, or custom orthotics. They may also be considered for selected patients with prominent bony pressure areas or those at increased risk for pressure-related skin breakdown. Every patient receives a comprehensive evaluation to determine whether dermal fillers or another treatment is the most appropriate option.
Are dermal fillers painful?
The procedure is performed in our office using local anesthesia to maximize comfort. Most patients experience little discomfort during the treatment itself. Mild soreness, swelling, or bruising may occur afterward, but these symptoms are usually temporary and improve over the following several days.
Can I walk immediately after the procedure?
Yes. Dermal filler treatment is performed in the office under local anesthesia. There are no incisions or stitches, and most patients are able to walk immediately after treatment. You'll receive personalized aftercare instructions before leaving the office, and most patients return to normal daily activities with minimal interruption.
How long do dermal fillers last?
The longevity of dermal fillers varies depending on the product used, the area treated, your activity level, and your body's natural response. In Dr. Stewart's experience, most patients enjoy the benefits of treatment for approximately 18 months, although some patients may experience a shorter or longer duration of effect. Because dermal fillers are temporary, repeat treatments may be recommended to maintain cushioning and symptom relief over time. Dr. Stewart will discuss realistic expectations and anticipated treatment longevity during your consultation.
Can dermal fillers help reduce the risk of foot wounds?
For selected patients with significant fat pad atrophy, prominent bony pressure areas, or a history of pressure-related skin breakdown, dermal fillers may help temporarily increase cushioning and redistribute pressure. As part of a comprehensive treatment plan that may also include custom orthotics, appropriate footwear, routine foot care, and off-loading when indicated, this additional cushioning may help reduce the risk of pressure-related skin breakdown and its associated complications. Reducing excessive pressure is an important part of helping protect vulnerable skin, particularly in patients at increased risk for ulceration.
Are dermal fillers a replacement for custom orthotics?
No. Dermal fillers and custom orthotics serve different purposes and are often used together. Custom orthotics help improve foot mechanics and redistribute pressure during walking, while dermal fillers temporarily increase cushioning beneath painful or high-pressure areas. Depending on your diagnosis, our doctors may recommend one treatment or a combination of both to provide the best long-term outcome.
Can dermal fillers replace foot surgery?
For some patients, yes. When conservative treatment no longer provides adequate relief, dermal fillers may offer a minimally invasive alternative before considering surgery. However, if your symptoms are caused primarily by a structural problem such as a bunion, hammertoe, arthritis, or instability, surgery may provide the best long-term solution. Every recommendation is individualized based on your diagnosis, symptoms, activity level, and treatment goals.
Are dermal fillers FDA approved for the feet?
No. While several dermal filler products are FDA approved for certain medical and cosmetic uses, they are not specifically approved for injection into the feet. Physicians may use FDA-approved products for off-label indications when, in their professional judgment, they believe the treatment is appropriate for an individual patient. If dermal fillers are recommended, our doctors will discuss the expected benefits, limitations, alternatives, and potential risks before treatment.
Will insurance cover dermal fillers?
Dermal filler treatment for the foot is generally considered an elective procedure and is not covered by most insurance plans. Our office will review the anticipated costs with you before treatment so you understand your options and can make an informed decision.
Why doesn't everyone with ball of foot pain need dermal fillers?
Ball of foot pain can result from many different conditions, including fat pad atrophy, stress fractures, neuromas, arthritis, plantar plate injuries, tendon disorders, and structural deformities. Because these conditions require different treatments, every patient begins with a comprehensive evaluation to determine the underlying cause of their symptoms. Dermal fillers are recommended only when they are expected to provide meaningful benefit based on your diagnosis and treatment goals.
