Stewart, DPM has provided laser treatment for toenail fungus since 2011 and was among the earlier podiatrists in Maryland to incorporate this technology into clinical practice.

What is laser treatment for toenail fungus?

Laser treatment is a non-invasive, in-office procedure that uses focused light energy to treat fungal organisms beneath the nail. It requires no injections or anesthesia and no downtime, allowing most patients to return immediately to their normal daily activities.

Laser therapy may be used alone or combined with topical or oral antifungal medications, depending on the severity of the infection and the individual patient.

How does laser treatment work?

The laser delivers focused light energy through the nail plate to reach fungal organisms beneath the nail. The goal is to reduce the fungal burden so healthier nail can gradually replace the damaged nail as it grows out.

Because toenails grow slowly, visible improvement occurs gradually over several months.

Why choose laser treatment?

For carefully selected patients, laser treatment offers a non-invasive treatment option without systemic antifungal medication.

Unlike oral antifungal medications, laser treatment acts directly on the affected nails without exposing the entire body to medication. It may be particularly appropriate for patients who cannot take oral antifungals, are concerned about potential medication interactions or side effects, or prefer an office-based treatment without downtime. Laser therapy may also be combined with other treatments when appropriate.

What to expect during treatment?

Treatment is performed in our office and typically takes only a few minutes per foot. Most patients experience a mild warming sensation during the procedure and return immediately to their normal daily activities without restrictions.

Because toenails grow slowly, visible improvement occurs gradually as healthier nail replaces the damaged nail over time. Patients are treated a total of 4 times over the course of 6 months, with a 2-month interval between treatments.

During treatment, patients use topical antifungal treatment for the nails and skin, along with an antifungal shoe spray, to help address potential sources of reinfection.

What to expect after treatment?

Our doctors recommend lifelong antifungal treatment of the skin to help keep athlete’s foot (tinea pedis) under control, since fungus on the skin can serve as a reservoir for recurrent fungal nail infections.

How do I know if I really have toenail fungus?
Not every thick, discolored, or damaged toenail is caused by a fungal infection. Trauma, psoriasis, eczema, aging, and other nail disorders can look very similar. Before recommending treatment, we perform a thorough examination and, when appropriate, may recommend laboratory testing to confirm the diagnosis.
Is laser treatment effective for toenail fungus?
Laser treatment can reduce the fungal organisms within the nail and create a healthier environment for a new nail to grow. Success depends on several factors, including the severity of the infection, the amount of nail damage, your overall health, and your commitment to preventing reinfection. No treatment can guarantee a cure for every patient.
Is laser treatment painful?
Most patients tolerate laser treatment very well. You may feel a brief sensation of warmth during treatment, but anesthesia is typically not required, and patients can return to normal activities immediately afterward.
How many laser treatments will I need?
Our standard treatment protocol consists of four laser treatments scheduled every two months over a six month period. This schedule is designed to repeatedly target the fungal organisms while healthy nail gradually grows in. Because toenails grow slowly, improvement occurs over time, and it may take 9 to 12 months for the damaged nail to be fully replaced by healthy new nail.
Is laser treatment better than oral or topical medications?
There is no single treatment that is best for everyone. Laser therapy, oral antifungal medications, and topical medications each have advantages and limitations. In some cases, combining treatments provides the best opportunity for improvement. After evaluating your nails, we will discuss which option—or combination of options—is most appropriate for your situation.
Will laser treatment also treat the fungal infection on the surrounding skin?
No. Toenail fungus is frequently associated with athlete’s foot (tinea pedis), which affects the surrounding skin. While laser treatment targets the infected nail, it does not treat the fungal infection on the skin. Regardless of whether you choose laser treatment, oral medication, or topical nail medication, long-term treatment of the surrounding skin with a topical antifungal cream is an important part of reducing the risk of recurrence. We often recommend continuing topical antifungal therapy even after the nail has cleared to help prevent reinfection.
How long does it take for the nail to look better?
Improvement occurs gradually as the damaged nail grows out and is replaced by healthy new nail. Most patients begin to notice improvement within 3 to 6 months after starting treatment, while full results typically take 9 to 12 months as the nail continues to grow.
Can toenail fungus come back after successful treatment?
Yes. Even after successful treatment, fungal infections can recur because fungal organisms are common in the environment. Wearing breathable footwear, treating athlete’s foot promptly, maintaining good foot hygiene, and disinfecting shoes when appropriate can all help reduce the risk of recurrence. We also often recommend long-term use of a topical antifungal cream on the surrounding skin—even after the nail has cleared—to help prevent reinfection.
Is laser treatment safe?
Laser treatment is non-invasive and is generally well tolerated. During your consultation, we will review your medical history, examine your nails, and determine whether laser treatment is an appropriate option for you.
How much does treatment cost, and does insurance cover it?
Laser treatment for toenail fungus is generally considered an elective procedure and is not covered by most insurance plans. The cost depends on the treatment plan recommended for your individual condition. We will review all fees with you before treatment begins.
Am I a good candidate for laser treatment?
The best candidates are patients with confirmed fungal nail infections who want a non-invasive treatment option and understand that healthy nail growth takes time.
